Fees and Payment

I currently accept insurance payments as a provider for:

BC/BS.

If you do not have the above insurance, I have a fee schedule for out-of-network patients. I will provide you with the documentation that is required for out-of-network reimbursement from your insurance provider.

Office Hours

Monday - Thursday : Appointments are available from 8:00 a.m. - 7:00 p.m.

Friday: Appointments are available from 8:00 a.m. - 5:00 p.m.
